As a Clinical Supervisor you will:
Provide supervision and feedback to all clinical staff regarding direct patient care and documentation Assist the Clinical Manager in the completion of in-home annual competency and evaluations Facilitate case management process with staff members, physicians, and referral sources Supervise patient care schedules to ensure continuity and quality Monitor full time equivalent productivity expectation and staffs to meet company objectives Be responsible to ensure the use of the 4Ms (What Matters to the patient, Medications, Mentation, and Mobility) and provide Age-Friendly Care QualificationsQualifications Bachelor's degree in Nursing Current unencumbered State professional nurse license Must maintain a valid driver's license and good driving record Prior experience in a certified home care agency, including OASIS Proficiency in leadership skills The ability to assess patient needs and formulate individualized patient care plans to meet those needs Proficiency in Microsoft Office #HC Recommended Skills Assess Patient Needs Case Management Clinical Works Friendliness Health Care Home Care Apply to this job.
